Indigenous Patient Navigator

Overview: Under the general supervision of the Health Administrator, the Indigenous Patient Navigator will facilitate culturally safe transitions in care, specifically discharge planning for clients being discharged from a regional facility back home. It will improve access to quality care through a variety of providers, both clinical and non-clinical.
Preferred qualifications:
- A diploma or degree in health or social sciences is preferred. A combination of education and experience will be considered.
- Palliative Care for Front Line Workers in First Nations Communities training; Learning Essential Approaches to Palliative and End of Life Care (LEAP) an asset
- Minimum of three years' experience working with First Nations, Inuit, and Métis communities in roles such as case management, health promotion, public health, or social work.
- Strong verbal and written communication skills.
- Superior interpersonal, collaboration and facilitation skills.
- Ability to support and facilitate engagement processes that build mutually supportive relationships with Constance Lake First Nation members and healthcare providers.
- Ability to work effectively within a multidisciplinary team
- Extensive knowledge of Indigenous cultures, languages, customs, and traditions.
- Ability to speak an Indigenous language is an asset
- A valid Ontario Driver's ''G'' license and access to a reliable vehicle.
- Current First Aid/CPR certification.
- Successful candidates must undergo drug screening & CPIC before beginning
- Willingness to participate in Indigenous cultural competency training
Duties and responsibilities:
- Serve as a liaison between Indigenous patients, their families, and healthcare providers
- Collaborate with multidisciplinary teams to ensure holistic care.
- Building relationships across transdisciplinary teams and community partners to formalize processes that strengthen transitions from acute care to community and long-term care.
- Provide culturally appropriate support to patients and advocate for their needs within the healthcare system.
- Facilitate access to healthcare services, including traditional healing practices and ceremonies.
- Educate healthcare providers on Indigenous cultures, customs, and traditions to promote cultural safety.
- Support patients in understanding their health conditions, treatment options, and care plans.
- Maintain accurate records of patient interactions and progress.
- Participate in community outreach and health promotion activities.
- Coordinate continuity of care; facilitate discharge planning, collaborate with circle of care partners; assist seamless care transitions
- Medical referral clerk duties when required
- Additional duties as assigned by the Medical Services Manager in support of the medical service department.
